• PARENT/SCHOOL COMMUNICATION

    REPORTING STUDENT ABSENCES: Please call the office and leave your child’s name, teacher, and reason for absence. Be as specific as possible, especially when your child has a contagious illness.

    EMERGENCY CONTACT INFORMATION: It is important that your child’s emergency contact information is current. Please notify us immediately if there are any changes in telephone numbers or emergency contact persons. Changes can be reported to the main office or the health office.

     ALERT THE SCHOOL NURSE REGARDING ANY NEW INFORMATION ABOUT FOLLOWING:

    • Allergies
    • Injuries and injury restrictions
    • Illness
    • Medications
    • Special accommodations
    • Other medical issues concerning your child
