HEALTH SCREENINGS PROGRAM

Health screenings are mandated by the New Jersey Department of Education as listed below:

• HEIGHT, WEIGHT AND BLOOD PRESSURE: Yearly for all grade levels
• VISION (DISTANCE ONLY)- Pre-K, K, 2^nd, 4^th, 6^th, and 8^th grades
• HEARING : Pre-K, K, 1^st, 2nd, 3rd, and 7^th grades
• SCOLIOSIS SCREENING: 5^th and 7^th grades

If you choose to waive or select certain screenings, please notify the nurse. If screenings are waived, then parent/guardians must annually provide a copy of the received screenings from the child's doctor. If a waived choice is selected, the expense is at the responsibility of the parent/guardian.
